Patient Life Safety: Lessons Learned v2.0Are you at risk of a Criminal Conviction or a Lawsuit?

In 2008 a Nurse Manager in Oregon was sentenced to 19 months in prison for her role in the death of a senior when the senior died after falling out of a Patient Lift. A Nurse plead guilty to avoid the trial, while a Certified Nursing Assistant was also convicted in the same case.

In 2003 a woman in Connecticut received $1.3 Million in her personal injury lawsuit after getting a compound fracture of her left tibia after falling from a Patient Lift.

In 2002 two workers received $1.05 Million in Massachusetts after being injured using a Patient Lift.

What are the odds of a similar incident from occurring?

Since 1981, (to 1997) Health Canada has received reports of 11 deaths and over 50 injuries related to the malfunction, failure or misuse of patient lifts in Canada. In the United States, there have been over 41 deaths and 443 injuries reported to the US Food and Drug Administration since 1985 (to 1997).

Solution

Due to the seriousness of both criminal convictions in Oregon and the above mentioned lawsuits, we significantly upgraded our course on Patient Lift Safety, which is now a 2 CD ROM set.

Patient Lift Safety: Lessons Learned v3.0 (CD-ROM)

The first CD-ROM contains:

  • In "Chronicles of a Hazardous Facility", you'll learn what happened when two residents and one staff member were injured in a Long Term Care facility over a four month period and how each incident of a resident falling out of a Patient Lift could've been prevented. The employer had to pay out approximately $20,000.00 extra in wages alone to cover shifts while the injured worker was on light duties and going for regular Physiotherapy treatments for approximately 7 months.
  • In "Chronicles of a Hazardous Facility 2", you’ll learn how the management team completely ignored what happened in their facility and were repeating the same mistakes 2 years later.
  • Two Ontario Coroner's Inquests which contain recommendations on how to prevent similar deaths involving Patient Lifts.
  • A Health Canada Report which contains several recommendations on how to prevent injuries and deaths involving patient lifts.
  • 2 documents on Patient Lifts from the Washington State Department of Labor and Industries
  • 3 A's of Patient Lift Safety.
  • 5 Rights of Patient Lift Safety.
  • 2 Lawsuit cases to emphasize the liability aspect of Patient Lift Safety.
  • 1 Criminal Convictions case.
  • 2 Health Canada alerts pertaining to Patient Lifts (2006 & 2008).
  • 1 Ontario Coroner alert pertaining to a Patient Lift in 2008.
  • And much more.

In addition, when this CD-ROM was being developed, actual scenarios were re-created to assist the learner in their understanding of Patient Lift Safety. An example of this is shown in the screen shots from the CD-ROM.

This CD-ROM is user friendly with easy navigation and takes approximately 3 hours to go through.

The Second CD-ROM contains:

The audio files of an actual criminal trial are located on this CD-ROM. In this trial you'll hear how a resident fell out of a Patient Lift and died in Hospital a few days later. One nurse plead guilty to avoid the trial. A Certified Nursing Assistant as well as the Nurse Manager stood trial and both were convicted. The Nurse Manager was sentenced to 19 months in prison. The trial covers such areas as:

  • How the fall could've been prevented
  • How the death of the resident could've been prevented
  • The importance of Incident Reports
  • The importance of notifying family
  • Doing proper assessments
  • Defining a fall
  • And so much more

This second CD-ROM contains the testimony of more than 20 witnesses.

By learning the numerous "Lessons Learned" from a multitude of sources contained in this revised 2 CD ROM set, you can drastically reduce the potential for injuries (and subsequent costs) and deaths associated with Patient Lifts and other Patient Transfer Devices.

Testimonial:

"Received the Patient Lift Course today and I've just gone through the first disk. I think it is excellent. I really liked the pictures and slides as a format of study. The most interesting thing about the format is that it mandates all nurses to be aware of which is the right sling for the right patient, of which I was not aware of before.

This course must become a mandatory teaching tool for all staff in Nursing Homes, Chronic Care, or any other facility that uses Patient Lifts."
